Mobile Giving Guidelines

Thank you for your interest in conducting a charitable campaign through the Innogive Foundation (“IGF”). In order to participate in carrier billed mobile fundraising you must complete and submit the following with the requested supplemental documents below. Be sure to also review your Mobile Application Service Provider Agreement (“MASP”) terms of service carefully.

If you have any further questions, please contact your MASP account representative. Applications are generally processed within two (2) weeks of IGF's receipt of all required documents. This application is valid for ninety (90) days.

1. Completed and executed Initial Application;
2. Copy of previous FY 990. If 990 not available then latest audited financials. Only Organizations with gross revenues in excess of $500,000.00 are eligible to apply for Carrier billed donations.
3. Copy of IRS Determination Letter;
4. Copy of Mobile Application Service Provider Agreement/Acknowledgement;
5. Copy of Bylaws (Amended/Restated if applicable);
6. Copy of Articles of Incorporation (Amended/Restated if applicable);
7. Copy of completed, signed and dated IRS Form W-9;
